HIPAA Breach Notification
This notice provides you with important information which occurred on and became evident late in the evening of June 29, 2026.
Brief Description of Breach:
On June 29, Taylor Psychological Services experienced a Breach of certain protected health information as a result of clicking a link and providing the practice phone number. The phishing attack led to a cell phone, email, and laptop being compromised.
Types of Information Compromised:
As a result of the attack, your name, contact information, emails and attachments, some introductory paperwork, and some financial information (i.e., checking account number and routing number), if emailed or scanned, could have been exposed. Your introductory paperwork would consist of the forms you completed before your first appointment including your health insurance form. Please note: After consulting with a cyber security expert and Therapy Notes (i.e, electronic medical records system), there does not appear to be any evidence that your electronic record was accessed.
Protect Yourself:
Provided below is the contact information for three major credit bureaus. Taylor Psychological Services recommends that you take the following steps:
Call the toll-free# of any of the three major credit bureaus listed below to place a fraud alert on your credit report. This can help prevent an identity thief from opening additional accounts in your name. As soon as the credit bureaus confirm your fraud alert, the other two credit bureaus will be notified to place alerts on your credit report and all three reports will be sent to you free of charge.

Order your credit report. By establishing a fraud alert, you will receive a free copy of your credit report. When you receive your credit report, examine it closely to make sure there are no unauthorized transactions.
Continue to monitor your credit reports to ensure no unauthorized transitions have occurred.
Check your bank and financial information for charges or unusual activity.
Review any explanation of benefits from your insurance company to check for any unusual charges or suspicious activities.
Consider changing your password.

Investigation and Steps Taken to Reduce Harm In The Future:
Taylor Psychological Services has taken several measures to investigate this breach including consulting with multiple IT specialists, Therapy Notes (i.e, the electronic medical record system), cell phone company, as well as with Xfinity and Verizon to verify the extent of the breach.
In addition, Taylor Psychological Services has taken the following steps to prevent a breach from occurring in the future:
A cyber security specialist has been engaged to setup my new devices and establish safe and secure ways of communicating. This process has included updating multiple security measures.
A new laptop and cell phone have been purchased. Taylor Psychological Services is currently using a temporary office for administrative work to insure a clean safe Wi-Fi.
Taylor Psychological Services has a new phone number and email address. Passwords for the computer and phone have been changed.
Links from unknown users will not be clicked.
Taylor Psychological Services will continue to work with the cyber specialist to insure your information will be secure in the future.
